24.09.080 Report of findings.

(1)    Whenever an inspection of a food establishment is conducted, the findings shall be recorded on a food service establishment inspection report form approved by the Washington State Department of Health and meeting the requirements of WAC 246-215-181 or as may be amended. This form will be completed for every site inspection or consultation.

(2)    The completed inspection report form shall state specific violations found, a summary score based on the number and type of red high risk factor and blue low risk factor violations, and a specific and reasonable period of time for correction.

(3)    At the conclusion of the inspection, a copy of the completed food service establishment inspection report form shall be furnished to the permit holder or person in charge and a signed acknowledgement of receipt requested.

(4)    If there is refusal to sign the acknowledgement of receipt, Clark County public health will inform the person that:

(a)    An acknowledgement of receipt is not an agreement with the findings;

(b)    Refusal to sign will not affect the permit holder’s obligation to correct any violations noted within the specified time frame;

(c)    Refusal to sign will be documented in Clark County public health’s record for the food establishment; and

(d)    Make a final request that the person in charge sign an acknowledgement of receipt of the inspection findings.

(5)    Clark County public health shall treat the inspection report as a public document and shall make it available for disclosure. (Sec. 9 of Ord. 2006-08-10)
