1.0 PURPOSE:

To establish a policy and procedure for employees to report obvious or potential safety hazards, as required by state law.

2.0 ORGANIZATIONS AFFECTED:

All departments/divisions.

3.0 REFERENCES:

WAC 296-24-040.

4.0 POLICY:

The Hazard Reporting System is part of the City’s accident prevention program. It is every employee’s responsibility to report an obvious or potential safety or health hazard.

5.0 DEFINITIONS:

5.1    An Obvious or Potential Safety or Health Hazard:

Any unsafe condition, either in a facility or on equipment, or any unsafe act or practice by employee(s) that could cause injury or illness.

6.0 PROCEDURES:

6.1    An employee who feels that a safety hazard exists fills out the top portion of the Safety Hazard Report form (Personnel Form #10F-29), which can be obtained from the division’s safety representative or from the Personnel office. The employee may remain anonymous if he/she wishes. The form should be returned to the safety representative.

6.2    Department/division safety representative keeps the yellow copy and distributes other copies as follows:

Department/division head - white copy

Employee filling out form - pink copy

Safety Coordinator (Personnel) - gold copy

6.3    The second part of the form is to be completed by the department/division head after the hazard has been investigated and action taken.

6.4    The completed form is then forwarded to the Safety Coordinator (Personnel) for discussion at the next Safety Committee meeting.
